Risk Assessment Checklist
It is a Her Majesty's Theatre requirement that any performance on stage that has the following: ladders, bikes, scooters, throwing or performing in front of the curtain, requires a special permission to be granted prior to the performance. In order to receive this permission a RISK ASSESSMENT form must be completed and handed in to the South Street Office/HMT Technical Manager 2 weeks before the start of the Section. Possible discussion/preview with a Her Majesty's Theatre Technician may be requested prior to the performance.
In 2011 the final dates Risk Assessment forms will be accepted for the following disciplines are:
One Act Play Festival: Friday 5th August
Energetiks Dance Competition: Monday 5th September
Westpac Calisthenics Competition: Monday 29th August
